Care and Support at Home Service

The service will cover all client groups including but not limited to: Adults over and under 65, personal and social care and support with non-complex primary support needs of: • Dementia • Mental Health Conditions • Physical Disabilities • Learning Disabilities • Sensory Impairments • Autism • Substance misuse aged 18 or over.
