Bradford City, Districts & AWC CCGs: Home Oxygen Assessment and Review Service

This service will ensure that oxygen assessment and review is managed by a single team for the 3 CCGs and that oxygen using respiratory disease patients are provided with additional support to manage their conditions. The service is for adult ( 18years and over ) patients. It is understood that the majority of this care will be required and received by patients with COPD but includes adult patients receiving oxygen for any reason.
