WF - Commnity Falls Prevention Service

We require a provider of specialist health services for people with a learning disability resident in Tower Hamlets. The service will deliver high quality, personalised and safe services to meet the needs of people in transition, adults and older people with learning disabilities, and those who display behaviours that challenge.
