Contract for a St Helens Falls Prevention Service

St Helens Borough Council Public Health Team are looking to commission a borough wide multi-disciplinary Falls Prevention Service. The service will meet NICE Guidance providing appropriate assessment, identification of falls risk / reasons for fall, and then tailored intervention to manage risk.
