HCC Sept 16 The Provision of an Integrated Rehablilitation Bed Service

This service specification is for an Integrated Rehabilitation Beds Service for People over 18 requiring care home based rehabilitation to prevent hospital admission or to facilitate hospital discharge. Rehabilitation care home beds are currently provided under a range of models which will be replaced from 2017 with a single model.
