Waltham Forest Community Anticoagulation Service pilot

The Community Anticoagulation service Pilot will be provided under a Community Integrated care model and will run for one year from 1st April 2016 to 31st March 2017 (with the option to extend for up to 1 year whilst evaluation and procurement takes place).
