Rapid Response Domiciliary Care

Cheshire East Council is inviting applications from providers of domiciliary care to deliver a service that will complement our existing service providers, through delivery of a Rapid Response Service, with an enabling approach, to prevent unnecessary hospital admissions and support the prompt discharge of patients from hospital back to their homes and avoid unnecessary longer term care packages where appropriate.
