Medway Council are seeking an organisation to deliver the Connected Communities Project on the Hoo Peninsula. The Provider will deliver the intervention as set out by the Commissioner for the programme. They will be required to recruit a part-time SPLW to deliver the social prescribing intervention to socially isolated older people with an ME3 postcode.
