AQP Domiciliary Care and Support for adults with CHC needs in Buckinghamshire

NHS Chiltern Clinical Commissioning Group and NHS Aylesbury Vale Clinical Commissioning Group wish to ensure the provision of domiciliary care services for the population of Buckinghamshire to meet the needs of patients who are eligible for Continuing Health care and wish to receive care in the community.
