Market Consultation - Discharge to Assess Pathway Beds

Portsmouth City Council (PCC) wishes to work with a small number of care homes to develop a Discharge to Assess (D2A) pathway out of hospital. It is expected that approx. 10 beds would be required within the city. Our expectation is to have the beds spread across as few a number of homes as possible to enable most efficient use of the support services (Physio, OT, Nursing, Social Work) in managing the flow of people into and out of the beds.
