Social Prescribing Pilot

Funding from the Improved Better Care Fund and Public Health has been made available for a two-year pilot in Great Yarmouth. Partners from across Health, Social Care and the Borough Council have agreed a proposal which will see 'Living Well Connectors' engaging with local residents, finding out what matters to them and 'co-producing' a plan to link them with appropriate support as an alternative to, or to complement medical intervention.
