Community Frailty Service (Brent)

The service provides proactive, holistic care for frail patients and coordinates health and social care across Brent’s five neighbourhoods. Implementing the NWL CFCCO specification and framework at neighbourhood level is critical to ensure standardised, integrated, and effective frailty care. Rising frailty prevalence further drives the need for a coordinated, cost-effective approach to improve outcomes and sustainability.
